Medical

NOTE: Beginning January 1 2010, the Class 1 & 2 and Class 3 forms will no longer be accepted at any camps. You must now use the Annual Health and Medical Form. - In Camp Medication
Other Camping
- Campership Application
- Please fill out the form and check the box most appropriate for the campership you want.
- Each Cub or Scout may receive one campership per year.
- Please make sure your proof of income is attached with the application.
- Please black out Social Security Numbers on your tax forms.
- Make sure your unit leader and you have signed page 2 of the application.
- A committee will review all of the forms in mid May. Once the committee has reviewed all the applications, each applicant will be notified of the outcome.
- For a unit to comply with requirements to receive one free leader at Mataguay or Camp Fiesta Island summer camp, each scout applying for a campership must pay 50% of their camp fee by April 30th 2010.

Notes:
- Use either the Local or National Tour Permit, depending on the distance you are traveling to the San Diego - Imperial Council camp.
- You may also use a similar form provided by your unit.
- For permissable firearm activities at the various youth programs of Scouting, consult the Guide to Safe Scouting, specifically the section on Guns and Firearms.
